Social Worker Connect was developed with the goal of increasing access to services in 2014.

Today we are working with community agencies to improve the outcomes of patients by helping provide services in the community and preventing the need for higher more expensive levels of care. We believe that if people can receive the care they need in their homes and community everyone in the health care system benefits.
